The South Eastern Health and Social Care Trust in Northern Ireland began working with CareLineLive back in 2021 on a project to maximise resources and improve workplace efficiency in it's home care service which provides over 30,000 domiciliary care visits a week via a team of some 600+ carers and additional office staff. The Trust operates as part of Northern Ireland’s integrated Health and Social Care (HSC) system, where health and social care services are delivered together under a single structure and are free at the point of use. As a large public-sector organisation, the Trust operates within a complex commissioning, governance and accountability environment - making large-scale digital transformation particularly challenging. Prior to partnering with CareLineLive, the home care service was managed with traditional tools like spreadsheets and printed paper-based rotas delivered by post. The slowness and untimeliness of information flows resulted in patients being discharged from hospital but no carer attending a visit due to the rota not arriving in time. CareLineLive, the all-in-one home care management software provider, has announced a further expansion of its partnership with Mid and South Essex NHS Foundation Trust (MSEFT) that is resulting in freeing 60 beds per day of patients that would otherwise remain in hospital awaiting discharge. Most hospitals in the UK suffer from what is sometimes called ‘bed blocking’. Patients well enough to be discharged are unable to go home as they are waiting for social care packages to be put in place. This can result in long waits in A&E before patients are admitted to hospital wards and long delays for ambulances as they wait for a space to become available in A&E. MSEFT began dealing with the issue of delayed discharges in 2016 via a “Bridging Service” where it started to provide short term domiciliary care services directly to patients leaving hospital while waiting for the local authority to commission a longer, more sustainable care package. We spoke to John Walter from Mid and South Essex NHS Foundation Trust about how CareLineLive has helped to improve efficiency within the Bridging Service that he manages. What is your role within your NHS Foundation Trust? I am Head of Integrated Care at the Mid and South Essex NHS Foundation Trust; I look after the Bridging Service that uses CareLineLive and other services which are the Hospital at Home and Discharge to Assess. Could you describe the services you provide that are similar to providing domiciliary care? We support people being able to get out of hospital earlier and to avoid admission to hospital, by providing care to those who are experiencing a delay in getting social care. Three to four days is the average time that patients stay in our service. Bridging is there to support same day discharge in line with our Discharge to Assess policy that was bought in due to Covid – patients have to leave within 4 hours of being medically safe to do so. Project Manager Howell Hughes is involved from the tender stage with focus on security and software functionality. He works with customers for the life of the project to successfully implement our scheduling and care brokerage cloud software solutions with added focus on delivery of complex third party implementations such as ContrOCC and Microsoft Entra ID Active Directory.
Our training and onboarding team deliver Train the Trainer sessions on our scheduling solutions and, combined with our support service, provide online and phone help desk advice during the implementation phase to aid the configuration.
The Train the Trainer onboarding sessions are designed to create customer-based super users. All customers have access to our help portal with a comprehensive collection of knowledge and support articles and videos on how the solution works. These ensure smooth additional in-house training and support.
